הקהילה הטרנסג’נדרית היא קהילה מגוונת המורכבת מאנשים אשר חוצים ומאתגרים את גבולות המגדר. מחקרים קודמים הצביעו על שיעורי הדבקות ב HIV  ומחלות מין בשיעורים גבוהים, בצורה לא פרופורציונאלית לאוכלוסייה הכללית, ועל התנהגויות מסכנות מרובות, כגון: ריבוי פרטנרים מיניים, מין אנאלי ווגינלי לא מוגן, הזרקה של הורמונים פיראטיים, ועוד.

רוב הניסיונות שנערכו על מנת למגר תופעה זו בקרב הקהילה הטרנסג’נדרית, כגון: יישוג ברחובות ותמיכה, לא נמצאו מועילות להפחתת רמת הסיכון. מחקר קודם הראה כי התערבות בקרב אוכלוסייה זו מצריך התמקדות ייחודית בגורמים הפסיכוסוציאליים ובהקשר המיני שבו עלולה להתרחש הדבקה ב HIV. אנשים טרנסג’נדרים מתמודדים עם סטיגמה ואפליה, בד בבד עם קשיים האישיים. לעיתים, הצורך בקבלת אישור על זהותם המגדרית, הן מהסביבה והן מעצמם, עלול להיות חשוב יותר מאשר הצורך לשמור על בריאותם במגע מיני. סיכון בריאותי נוסף טמון בשימוש משותף במחטים להזרקת הורמונים. רמת הסיכון עולה ככל שהמערכת הרפואית פחות נגישה לאותם טרנסג’נדרים המעוניינים ליטול הורמונים.

בתוך ההקשר הזה, החוקרים פתחו מענה עבור הקהילה הטרנסג’נדרית: סמינר בן יומיים לקידום הבריאות בקרב טרנסג’נדרים. ההנחה היא שאנשים ידאגו לעצמם יותר כשהם מבינים את הסיכונים בהתנהגויות שלהם. בסמינר השתתפו 181 טרנסג’נדרים, בגילאי 18 ומעלה.

נראה כי ההתערבות המוצעת הצליחה להפחית עמדות שליליות של המשתתפים לגבי שימוש בקונדום ובהפחתת התנהגויות מיניות מסכנות. זוהי הערכת ההתערבות הראשונה שמצביעה על שיפור בשימוש בקונדום ובהרגלים מיניים בקרב האוכלוסייה הטרנסג’נדרית. ואולם, השיפור בהרגלים המיניים לא ניכר לאחר שלושה חודשים, כשנערכה מדידה חוזרת. החוקרים מניחים כי תופעה זו קשורה לחשיפה לסטיגמה ואפליה חברתית עמם מתמודדים טרנסג’נדרים.

רוב הטרנסג’נדרים במדגם לא סבלו ממחלות מין או HIV. רובם הגיעו ממרכז ארה”ב, שם שכיחות המחלות קטנה, ורובם גם לא עסקו בזנות. אולם, רובם סבלו מדיכאון, בדידות, אובדנות והתעללות פיזית או מינית בילדות. על כן, חשוב ליצור התערבויות גם לטרנסג’נדרים שאינם נמצאים בסיכון למחלות מין ו HIV, אך חשופים לסיכונים אחרים.
